Notes:
Include the initials and surname of all authors
Where there are multiple authors, use commas between each name, and precede the final name with 'and'
The title of the chapter or section that has been used should be given in quotation marks
Put the title of the entire book in italics
Edition number does not need to be given for first editions
If the place of publication is not well known, include the name of the relevant country or state too
Give page numbers unless the entire book is used.
